We offer a comprehensive lifestyle medicine evaluation. After assessing past medical history, family history and lifestyle habits, our lifestyle medicine team works with the patient to identify areas that the patient would like to improve.
Working together, we set health-enhancing goals, such as increasing physical activity, improving diet or reducing stress.
The lifestyle medicine team designs an action plan, which includes identifying resources to support your goals.

The lifestyle medicine team provides the support and encouragement patients need to attain and maintain their personal health goals. In doing so, current health concerns can be minimized and risk for future health problems reduced.
